Rework armour penalties

Following #951 and #987 it is time to reconsider the attack penalties for wearing heavy armour. The trivial to-hit penalties have always been ineffectual, and my suggestion is that we look at adding an amount of energy per blow which increases with heavy armour. See the discussion at

This would be a good opportunity to remove blows_table completely and use a continuous formula such as the one proposed by Eddie on Oook (need link). We could then add epb based on total weapon and armour weight (or just body armour + gloves + weapon if people prefer), and reduce this added factor for warriors, reduce it a little less for paladins, and leave it for the other classes.
