Make disturbance options better

"Disturb whenever any/viewable monster moves" should be replaced by "disturb when monster in LOS". Also, disturb when things with PASS_WALL and EAT_WALL are approaching when seen with ESP. "If you can hear grinding noises you ought to be able to be disturbed to avoid death to an approaching umberhulk."
"Move" should mean "is awake" for unmovable monsters so that you don't accidentally run next to Death Mold or Floating Eye.
The "player state change" and "boring things happen" options should have better names.
