It doesn't happen all the time, but I have successfully reproduced this.

I am pretty sure that either ensure_connectedness() is buggy, or that the cave squares aren't being initialized correctly in create_vault(), or both.

This is fixed in staging.

To confirm it, you need to generate levels until you find one where there are rooms on opposite sides of a vault, and a tunnel connecting them which goes around the vault. Or you can just play for awhile and notice things are fine :)
